چکیده

از بین بیماریهای قلبی، بیماری عروق کرونر قلب (CAD) علت اصلی مرگ و میر جوامع انسانی می باشد و جراحی پیوند دورزننده عروق تاج مانند قلب (CABG) متداولترین مداخله درمانی است. دوره بستری بیمارستانی پس از جراحی، تخمینی از میزان موفقیت جراحی به حساب می آید که کاملا توسط عوامل طبی و جمعیت شناختی قابل توضیح نمی‌باشد. بلکه عوامل روانشناختی نیز در این میان مهم می باشند. بنابراین هدف پژوهش حاضر این بود تا علاوه بر این عوامل، به بررسی تاثیر عوامل روان شناختی بر طول دوره بستری بیمارستانی پس از این جراحی بپردازد. از بین بیماران مراجعه کننده به مرکز قلب بیمارستان شهید رجایی تهران که کاندیدای این جراحی بودند، 150 بیمار (30 نفر زن و 120 نفر مرد) به روش نمونه گیری در دسترس انتخاب شدند. از بیماران خواسته شد پرسشنامه 21 سوالی DASS-21 را تکمیل کنند. تحلیل رگرسیون گام به گام متغیرهای روانشناختی، طبی و جمعیت شناختی نشان داد مهمترین متغیرها برای پیش بینی طول مدت بستری بیمارستانی پس از جراحی، اضطراب و افسردگی، دیابت و فشار خون بالا و سن و جنسیت هستند. نتایج نشان دادند که می توان با ارزیابی پیش از جراحی این متغیرها، به شناسایی بیمارانی نایل شد که ممکن است این نوع جراحی برای آنها موفقیت آمیز نباشد.

Abstract:
Of cardiovascular disease, coronary artery disease (CAD) is the main cause of death in human societies leads to coronary artery bypass graft surgery (CABG) as the most common intervention. The length of hospital stay after surgery is an estimate of the success rate of surgery that cannot be completely explained by demographic and medical factors. Therefore, the objective of this study was to investigate the impact of psychological factors along with medical and demographic factors on the length of hospital stay after CABG surgery. In this study, 150 patients (30 females and 120 males) were selected from the convenience sampling method, among patients referred to Tehran Heart Center of Shahid Rajaie Hospital who were candidates for CABG surgery. Patients were asked to complete the questionnaire of 21 questions for anxiety- depression- stress (DASS-21). Stepwise regression analysis for psychological, medical and demographic variables showed that the most important variables to predict the length of hospital stay after surgery were anxiety and depression, diabetes and high blood pressure, age and gender. Thus, the results showed that the use of pre-operative assessment of these variables in the clinical setting, we can identify those patients for whom surgery may not be successful.
