بررسی فراتحلیل عوامل روان‌شناختی بروز میگرن در ایران

چکیده

هدف: مبانی نظری و مطالعات انجام شده حاکی از عوامل خطر روان‌شناختی در بروز و تشدید میگرن است که شناسایی این عوامل می‌تواند در درمان و پیش آگهی بیماری مذکور تأثیر داشته باشد. لذا. پژوهش حاضر با هدف بررسی مرور نظام دار و فراتحلیلی بر عوامل خطر روان‌شناختی میگرن انجام گرفت. روش: مطالعه حاضر، فراتحلیل بود. بدین منظور، تمامی پژوهش‌های ایرانی انجام شده در زمینه عوامل خطر روان شناختی بیماری میگرن، به صورت نظام مند مرور و از میان آن‌ها، پژوهش‌های دارای ملاک‌های فراتحلیل، توسط نرم افزار جامع فراتحلیل بررسی شدند. از میان 240 مطالعه مرور نظام مند شده، تعداد 29 پژوهش جهت بررسی مروری انتخاب و در نهایت 21 پژوهش که تمامی ملاک‌های مورد نظر را داشت، فرا تحلیل شدند. یافته‌ها: نتایج نشان داد که متغیرهای روان‌شناختی باورها، عواطف و اختلالات خلقی، شخصیت، استرس و راهبرد های مقابله ای، حمایت اجتماعی و کیفیت خواب با بیماری میگرن مرتبط اند. به نحوی که اندازه اثر باورها با میانگین 95/1؛ اختلالات خلقی 86/1؛ شخصیت 45/3؛ استرس و راهبرد های مقابله ای 76/2؛ حمایت اجتماعی 10/1 و کیفیت خواب 17/1 به عنوان عوامل خطر روان‌شناختی میگرن محسوب می‌شدند. میانگین اندازه اثر کلی مطالعات انجام شده برای مدل اثرات ثابت برابر با 43/17Z= و معنادار (000/0P=) و برای اثرات تصادفی برابر با 85/1 Z=و معنادار (049/0P=) بود. نتیجه گیری: با عنایت به نتایج حاصل به عنوان مقدمه ای برای شروع، ارائه یک پروتکل درمانی مبنی بر متغیرهای روان‌شناختی مذکور، جهت بهبود رفتارهای سلامت افراد میگرنی توصیه می‌گردد.

Aim: This study aimed to systematically review and meta-analysis of psychological risk factors migraines. Methods: This study was a meta-analysis. To this aim, the Iranian research’s in the field of psychological risk factors of migraine has criteria for a systematic review and was performed by software. The 240 study was a systematic review, 29 studies were selected to review and 21 studies that had all the required criteria, were selected for meta-analysis. Results: The results showed that the psychological beliefs, emotions and mood disorders, personality, stress and coping strategies, social support and quality of sleep were associated with migraine. Somehow that effect size of belief with average 1.95; mood disorders 1.86, personality 3.45, stress and coping strategies 2.76, social support 1.10 and sleep quality 1.17 were considered psychological risk factors for migraine. The average size of the overall impact studies for the fixed effects model was Z = 17.43 and significant (P =0.000) and the random effects was Z = 1.85 and significant (P =0.049). Conclusion: Considering the results as a prelude to starting, is recommended a treatment protocol based on the psychological variables to improve the health of migraine treatment.
